چکیده

SIR, We thank Dr Von Vollenhoven for his interest in our paper. The questions [1] he raises about our article that examined the cost–effectiveness of biologics using the British Society for Rheumatology Biologics Registry (BSRBR) [2] provide an excellent opportunity to describe why decision modelling should be used in calculating a majority of cost–effectiveness ratios. We refer to an article by Sculpher et al. [3], which eloquently explains many of these points in detail. It should be noted that while the title of the paper refers to trials, much of its content is applicable to a registry like the BSRBR. We summarize the key points:

Patients' perceptions of treatment with anti-TNF therapy for rheumatoid arthritis: a qualitative study.

OBJECTIVES Anti-tumour necrosis factor (anti-TNF) therapy is a highly effective treatment for rheumatoid arthritis (RA), as documented using standard outcome measures in clinical trials. Anecdotal experience suggests health benefits for patients other than those measured in this way. We wished to explore Patients' experience of and views about this treatment and the British Society for Rheumato...

Serum Levels of TNF-α, TNF-αRI, TNF-αRII and IL-12 in Treated Rheumatoid Arthritis Patients

Background: Rheumatoid arthritis (RA) is a chronic multisystem autoimmune disease common in all races and ethnics. Cytokines and cytokines receptors play an important role in RA pathogenesis and clinical presentation. Objective: To investigate the serum levels of TNF-α, TNF-α RI, TNF-α RII and IL-12 in RA patients and healthy control group. Methods: In this study 43 patients fulfilling the revi...

Venous thrombotic events are not increased in patients with rheumatoid arthritis treated with anti‑TNF therapy: results from the British Society for Rheumatology Biologics Register

OBJECTIVES Past studies have reported conflicting rates of venous thrombotic events (VTEs) in rheumatoid arthritis (RA). The current study aimed to compare (1) the rates of VTEs in patients with RA treated with anti-tumour necrosis factor (anti-TNF) therapy versus those treated with non-biological disease-modifying antirheumatic drugs (nbDMARDs) alone and (2) the rates between each individual a...

Cost-effectiveness modelling of biological treatment sequences in moderate to severe rheumatoid arthritis in France.

OBJECTIVES Modern treatment of RA includes the use of biologics. Their cost is high and comparison between different treatment strategies is needed. METHOD Direct medical costs of RA in France were evaluated based on expert opinion. Then, simulation-decision analytical models were developed to assess four biologic treatment sequences over 2 years in patients failing to respond to at least one...

Risk of septic arthritis in patients with rheumatoid arthritis and the effect of anti-TNF therapy: results from the British Society for Rheumatology Biologics Register

OBJECTIVES To evaluate the risk of septic arthritis (SA) in patients with rheumatoid arthritis (RA) treated with anti-tumour necrosis factor (TNF) therapy. METHODS Using data from the British Society for Rheumatology Biologics Register, a prospective observational study, the authors compared the risk of SA between 11 881 anti-TNF-treated and 3673 non-biological disease-modifying antirheumatic...
